Nearly one in five residents lacks health insurance — a number that doesn’t fully capture the larger group of people who are underinsured, between jobs, waiting for coverage to begin, or simply unable to afford the out-of-pocket costs that come with insurance that looks complete on paper but falls short in practice.
What most people don’t know is that Miami-Dade has 65 federally funded community health clinic locations — operated by roughly ten major organizations — that are required by federal law to see all patients regardless of insurance status or ability to pay. These are not charity clinics in the traditional sense. They are fully accredited primary care organizations with board-certified physicians, dental programs, behavioral health services, pharmacies, and laboratory facilities. Their costs are determined by a sliding fee scale based on your income — not a fixed price. For someone with no income, that can mean $0 to $30 per visit.

Every Federally Qualified Health Center (FQHC) in Miami-Dade operates on a sliding fee scale — a federally required pricing structure where your cost is determined by your income relative to the federal poverty level.
In practice: if your household income is at or below 100% of the federal poverty guideline (about $15,060 for a single person in 2026), your visit cost is typically $20 to $40 or less at most centers. Some charge as little as $0 for patients with no income. Above 200% of the poverty level, costs approach standard market rates, but no one is turned away for inability to pay.

Florida’s first federally qualified health center — founded in 1967, the fifth FQHC established in the entire country — JTCHS operates 16 facilities and 8 comprehensive primary care centers across Miami-Dade County. JTCHS also operates in 40 Miami-Dade public school locations, bringing primary and preventive care directly into schools — meaning children don’t need a separate appointment for routine health needs.

One of the largest FQHC networks in South Florida, CHI operates 13 health centers serving a predominantly Black, Hispanic, and Caribbean patient population across Miami-Dade and Monroe Counties. Services include family medicine, pediatrics, OB/GYN, dental, behavioral health, HIV care, and pharmacy.
CHI is one of the most linguistically diverse healthcare organizations in Miami — services are available in Spanish, Haitian Creole, and other languages commonly spoken in Miami-Dade’s communities.

A not-for-profit FQHC operating since 1970, Banyan Community Health provides integrated primary medical care and behavioral health services across Miami-Dade and Broward Counties. What distinguishes Banyan from other Miami-Dade FQHCs is its explicit integration model — primary care and mental health services are delivered in the same setting, with the same care team aware of both. This is particularly important for patients managing chronic conditions alongside anxiety, depression, or substance use.
Banyan has Patient Centered Medical Home (PCMH) designation and multiple locations across Miami-Dade including Little Havana, Cutler Bay, and additional sites.

Camillus Health Concern is Miami-Dade’s dedicated Healthcare for the Homeless FQHC — one of the few health centers in the country established specifically under the 330(h) Healthcare for the Homeless grant program. Operating since 1989 and designated as an FQHC in 1996, Camillus serves adults experiencing homelessness with primary care, behavioral health, case management, transportation to appointments, and free medications for those with no other resource.
Camillus is co-located with Camillus House, Miami’s largest provider of shelter and homeless services — meaning patients have access to healthcare, food, shelter, and support services in an integrated environment.
A photo ID is not required for patients who do not have one. The sliding fee scale goes as low as $0 for patients with no income.
